When you enroll in Aetna Select EPO, you must select a primary care physician (PCP) for yourself and each covered family member. Your PCP provides basic, routine care and referrals to specialists. Benefits are paid only when your PCP provides or refers your care (except for certain emergency care services). If you seek care on your own, you pay the entire cost.
Most preventive care services are covered at 100% after you pay a copay. The majority of other services are covered at 90%. There is no deductible with Aetna Select EPO.
Every year there is a limit to the amount you have to pay out of your pocket for covered expenses:
- In-network out-of-pocket maximum: $1,500 individual/$4,500 family
If you reach this amount, the plan will pay 100% of covered medical expenses after you pay any applicable medical and prescription drug copays.
