Increasing Coverage or Electing Coverage For the First Time
You can increase your life insurance coverage during open enrollment. Some increases, however, may be subject to evidence of good health -- also referred to as Evidence of Insurability (EOI). If you are electing life insurance coverage for the first time, your request will also be subject to EOI. (This does not apply to Child Life.)

Apply online for EOI – no forms!
If you are required to submit an EOI application you will receive a letter from Aetna with instructions for that process. You must apply within 30 days of receiving this letter. To ensure that you complete this process on time, you are encouraged to complete the application online. If necessary, you may still mail your application in the envelope provided.

If you have any questions, please call 1-800-660-9913 to speak to an Aetna representative in the EOI unit.
